About Koi Medical

Koi Medical is a membership-based practice, although we do offer non-membership services.  Our membership plans offer access to our expert healthcare provider.  Our membership plans are designed to provide you with the highest level of healthcare flexibility and convenience.  A one-year contract is required for Primary Care Membership Plans and cannot be cancelled prior to the end of the one-year contract agreement term.  Primary Care Membership Plan contracts will be renewed annually.  Otherwise, there are no contracts required for all other membership plans and services, and you're free to cancel non-Primary Care Plan memberships at any time without penalty, as long as services being provided by Koi Medical have ended and there are no further agreed upon requirements for continued charges by Koi Medical.  

 

Koi Medical does not accept insurance or submit claims to insurance for reimbursement.  Although we do not accept insurance, we work with clients who are insured or uninsured.  Koi Medical can provide a superbill for an insured client to submit a claim to their insurance company for reimbursement, however, there is no guarantee of reimbursement from the insurance company for any services provided by Koi Medical.

Direct Patient Care Model

Koi Medical was established based on a Direct Patient Care model.  A Direct Patient Care model is a healthcare delivery approach that focuses on providing personalized, patient-centered care through a direct relationship between a healthcare provider and the patient.

At Koi Medical, you will have a dedicated healthcare provider to be your primary contact.
